Learner Pathway Timeline

All progress reports, parent session feeedback forms, goal setting session & weekly session reports will be sent to comissioning school throughout academic year.

1st Week

Making contact Making contact with parents/careers to introduct Lets Make Change & find out how we can best support

Building up a picture Reviewing previous education plan of the learner to understand how we best support

Touching base Making contact with commissioners to let them know our next steps

2nd Week

Lets meet Tutor to carry out a visit to meet learner & speak about what education looks like with LMC

Putting a plan together LMC will devlop a bespoke plan to deliver with learner that consider subjects studied, interest & hobbies, SEND needs & all encompassing aspects that support progress in education

3rd Week

Wishes & feelings Tutor starts delivering sessions by collecting the learner's wishes & feelings to ensure their voice is considered in educational sessions.

Acadmeic Assessments Tutors will assess the learning levels of learners in core subject areas

1st Term

Progress LMC will complete learner termly report detailing how learners have progressied with provision & plans for next term

Goal setting Tutors to sit down with learners identifying future goals both student & tutor would like to achieve

Catch up Time to arrange a catch up with parents/carers to discuss how things are going & allow opportunity to speak about plans for the next term

3rd Term

Core subject qualifications Learners to obtain core subject qualifications in GCSE or FS

Academic end of year progress report Sharing a picture of the full year of progress that learners have made with LMC

End of year student celerbration Lets celebrate the success achieved in another academic year. Allowing students to pick a session or trip to celerbrate the end of the academic year
